I attribute most of my losses to poor play on my own self, not anybody else, but I know that's not a popular opinion when it comes to reasons lost. As somebody who works hard to grind out and improve, I rarely feel that games are unwinnable from some hyper enemy smurf. Smurfs are also not that commonplace, I'd say far below 10%. I'd say over my 2000 games around 5-15% have been completely out of my control (somebody running down mid) which is much less than people make it seem. A lot of the toxicity is real (griefers, toxic players, even smurfs) but it's honestly a minority of games. I started playing again two years ago and went from 2.6k - 6.5k.
I've played against Immortals in unranked and they play entirely differently than when playing ranked.
